Respirator Fit Test

Dear Grower:

According to OSHA requirements, growers that apply soil fumigants need to get medical clearance to certify that they are fit to wear a respirator and to have their respirator fit-tested every year, before they can apply fumigants. The Person County Extension Service has partnered with Occupational Team Solutions in an effort to complete these requirements.

On Monday, January 25, 2016, respirator fit-testing will be done at the Granville County Extension Office, 208 Wall Street, Oxford, from 8 A.M. – 5 P.M.

You first need to complete the enclosed OSHA Respirator Medical Evaluation Questionnaire. Each person to be scheduled for respirator fit-testing must complete and return the OSHA Medical Questionnaire as soon as possible to get the process quickly started and mail it right away to:

Delaying sending in the questionnaire will make it more difficult to get you a fit- testing appointment on January 25.

 

OTS will then facilitate the Medical Clearance Review of the required OSHA Medical Evaluation Questionnaire with Amelia Wilson, MD.

Upon review and clearance by Amelia Wilson, MD., each person not qualified for respirator-fit testing will be notified and should not schedule a fit test with the Granville County Extension Office.

Those that have been given clearance will not be notified by OTS but need to call the Extension Office at 919-603-1350 to schedule your fit-testing appointment three days after mailing the completed OSHA Medical Evaluation Questionnaire to OTS. If you have not been able to complete the questionnaire and mail it to OTS, bring the form with you after making your appointment at the Extension office and it will be reviewed on January 25th. You must not eat, drink, or smoke at least 30 minutes before testing. The test  takes 30 minutes to complete. The test includes pulmonary function (required), blood pressure check (required), and the respirator mask fit-test.

If you are being fitted for the first time, do not purchase a respirator before you get tested, as it might not be the best fitting respirator for you. There will be three different brands, models, and sizes available to select from at the Extension Office, after your test is completed.

Other growers who currently have a respirator, and have an appointment to be re- tested on January 25, must bring their respirator along with the filter cartridges with them. Each person who successfully passes the respirator fit-test will be given a certificate.

The cost of the test is $75.00, which includes Physician review of the OSHA Medical Questionnaire, Pulmonary Function Test, and Respirator/Mask Fit Test. The cost is the same for new people being fitted for the first time, as well as people coming back every year to be re-fitted.

The NC Agromedicine Institute along with a grant from the NC Tobacco Trust Fund Commission will reimburse growers for half of the cost of a respirator, and half the cost of the test ($37.50). Growers can fill out a form for reimbursement at the Extension Office after their fit-test is completed.

The payment method is cash only; no checks or credit cards will be accepted. Please make every effort to bring the exact amount. You will receive a receipt to attach to the cost share reimbursement form at the Extension Office.

Give Gwen a call at 919-603-1350 to schedule your fit-testing appointment, three days after you have mailed your questionnaire to OTS. First come, first served.

Remember that fit-testing will only be done in Granville County on Thursday, January 25.
